We participate with many insurance companies for
submission and payment of your medical expenses. You will be asked to
supply us with the appropriate billing information and we will make a photocopy
of your insurance card. In instances of self pay we accept check and
cash payments as well as Mastercard/Visa. Discussion of our fees for any
procedure will be provided when requested. Our patient accounts
representatives will be happy to assist with submission of claims and
